Josh there is something that is triggering this. IV checker, 3rd party software or some other factor. Investigate stuff on your end and trust that if you are following the rules or start following them your account will be fine.

The Only App That I’m Using In Conjunction With The Game Is PokeGenie. Also, Niantic Did Respond On Twitter To Me, But They Just Sent A Link To Their Useless Customer Service Form Site

If your Trainer name is the same as your forum name it would be a reasonable assumption that someone may have reported your account. I did that to a player whose name was a racial slur. Could have been someone who saw your avatar in a gym and reported? Hope it works out, I’m sure within 2 weeks it will be back to normal
